While reviewing applications, please keep the
AstraZeneca ACT on Health Equity goal central:
To advance health equity for historically excluded and disenfranchised populations through community-based programming that prioritizes their social, cultural and linguistic needs.
You will be required to submit one scorecard per application assigned to you and will be able to access these applications for review on Box. This year, each application will be scored by two reviewers, and we will combine both scores to determine finalists.

Scoring guidance:
Please familiarize yourself with the full scorecard first to facilitate your review of each application. Questions within the scorecard vary in scoring range based on importance and relevance. Each application is eligible to receive a total score of up to 73 points.
To support your review, we’ve provided references to key questions within the application that may apply, but relevant information may be spread throughout.

- We will need your support to ensure that scoring is assigned to the correct application. Please be sure to verify all your responses and ensure the correct organization and program name are outlined before submitting.
- To help us identify the most impactful programs, please follow scoring guidance closely. Top scores should be reserved only for exemplary programs that outline strong approaches, clearly defined achievable S.M.A.R.T. goals, and give confidence that their program will truly drive long-term and sustainable change.
- When reviewing and scoring, use the notes sections to share your rationale/thinking for later review – the more detail the better.
- Do not discuss or compare applicants/applications with one another. You should review each submission independently and objectively.
- Please proactively recuse yourself from any application where there may be a conflict of interest.
