2024 ACT on Health Equity: Community Solutions Challenge

The AstraZeneca ACT on Health Equity: Community Solutions Challenge is advancing health equity through the support of community-based nonprofit programming that prioritize the social, cultural, and linguistic needs of historically excluded and disenfranchised populations. Organizations may apply for $30,000 to fund new or existing programs.

Applications must meet the following general criteria:

  • Program Focus: Programming must focus on one of the following areas:
    • Nutrition: Address nutrition as a social determinant of health, including reducing food insecurity and providing comprehensive nutritional education to improve health outcomes.
    • Healthcare Access: Improve access to quality healthcare by addressing barriers, including health education, access to preventive screenings, and transportation services.
    • STEM Education: Expand educational and career readiness opportunities in science, technology, engineering, and mathematics for underserved communities.
    • Oncology: Advance cancer care for underserved communities through innovative solutions to provide more equitable access to quality screenings, treatment, personalized care, and resources for people living with cancer, caregivers and loved ones.
  • Community Focus: Programs that address health disparities among historically excluded and disenfranchised populations and prioritize their social, cultural, and linguistic needs.
  • Program Footprint: Nonprofit organizations across the US and US territories are invited to apply for programs benefiting one or more local communities. While not a requirement, special consideration will be given to programs that reach AstraZeneca footprint geographies (California, Delaware, Georgia, Maryland, Massachusetts, New York, Ohio, Pennsylvania, Texas, and Washington)

2024 Community Solutions Challenge Scorecard

Thank you for serving on this year's Review Committee! We will count on your support in assessing and identifying application finalists for the 2024 ACT on Health Equity: Community Solutions Challenge.

While reviewing applications, please keep the AstraZeneca ACT on Health Equity goal central:

You will be required to submit one scorecard per application assigned to you and will be able to access these applications for review on Box. This year, each application will be scored by two reviewers, and we will combine both scores to determine finalists.

Scoring guidance:

Please familiarize yourself with the full scorecard first to facilitate your review of each application. Questions within the scorecard vary in scoring range based on importance and relevance. Each application is eligible to receive a total score of up to 73 points.
